What is the difference between form design and template design?

FORM DESIGN – You can count on us to provide quality form design, drawing on our experience to recommend the best format for your survey questions. For instance, we suggest that you always put the highest value (or best response) first. For larger surveys, we may recommend laying out the form on 11 x 17 sheets instead of on four 8.5 x 11 sheets, eliminating the need to staple the sheets. Recommendations such as these will save you money in printing, preparation and processing time.

 

FORM TEMPLATE DESIGN – Form template design indicates schema output, whether on an existing form or one we have designed for you. This tells our software where to look for marks and data to be recorded to the database.

OMR fields are normally given an alpha or numerical value. For instance, the question may ask “Are you Male or Female?" The value assigned to "Male" in that field would be "1," and the value assigned to "Female" would be "2."

OCR and ICR fields are normally set up as barcode or alpha and/or numerical fields. We can provide rules, look-ups and scripting for any part of your survey.
